Entry from delegates can be submitted by downloading the ipmr-cme-brochure.pdf file. The brochure contains the Registration Form and an Accommodation Form. Delegates should also download the invitation-letter.pdf file.

The delegate should fill in the Registration Form accordingly and send the completed form duly filled in along with the registration fee (DD) to the Organizing Secretary concerned. The registration fee is Rs. 500/- per delegate and Rs 300/- per delegate, if the delegate is a Post Graduate student. The registration fee is to be paid in the form of a Demand Draft in favour of "Organizing Secretary, IAPMR Mid-Term CME 2008" and to be payable at Imphal.

For out-station delegates who are seeking to opt for accommodation, it is requested to fill in the Accommodation Form with the preferences marked accordingly as given in the form. This Accommodation Form has to be accompanied by a Demand Draft of Rs. 600/- as advance hotel charges for the first day. The actual accommodation / hotel charges are to be paid to the Organizing Secretary after arriving at the conference venue. The accommodation fee is to be paid in the form of a Demand Draft in favour of “Organizing Secretary, IAPMR Mid-Term CME 2008” and to be payable at Imphal.

Every delegate who intends to present their papers related to the theme of CME is required to submit the abstracts in duplicate form in A4 size papers with double spacing and 2 cm margin. The abstract should also be sent across in a CD. The abstract contents with CD should reach the organizers latest by July 15th 2008.
